I would use Academic Screeners for Reading, Writing, and Mathematics. After that, I would examine school records and get in contact with teachers (with parent permission). I would then use this data to design an individualized learning plan for each child designed to fill in educational gaps/holes that may lead to academic difficulites. I would also additional supportive websites that can be used to monitor/track progress.
